We know that the decision to join the Fairway community is a life-changing decision.
A decision only reached after much deliberation, research and consultation. Our commitment to you extends beyond quality of care to your overall wellbeing. We offer a vibrant, safe living environment that allows you to continue living as you choose, with a little bit of extra help.
To find out whether you’re eligible to join the Fairway community – either permanently or for day respite care – please complete the three-step application process below.
Peace of mind
The government’s Aged Care Assessment determines suitability for government-subsidised residential aged care homes. This assessment is conducted by mobile ACAT teams.
Ask your GP to arrange an assessment, or book one via telephone on 1800 200 422 or at www.myagedcare.gov.au.
Assessments can take several weeks to be finalised, so be sure to organise it early. An ACAT assessment is required for entry into Fairway Bayside Aged Care’s permanent residential care.
Income and assets
The government requires a contribution towards care provided from those who can afford to do so, and will provide a subsidy for those applying for financially supported permanent residential care.
Prospective residents are required to complete and submit Permanent Residential Aged Care form – Request for a Combined Assets and Income Assessment (SA457) here to assess the applicable Means Tested Care Fee or eligibility for a government supported place.
For low means residents, Fairway Bayside Aged Care requires a copy of the government response letter to this form before your application can be processed. For all other applicants, the government requires receipt of the completed form within 28 days of admission or penalties will apply.
